Access to Care Survey

This survey is being run by the St John's Medical Centre Equity Team. It will help us understand how patients like to engage with health services, preferred ways of communicating and any barriers patients face when trying to access primary health care at St John's Medical Centre.

This is a short survey for any St John's Medical Centre patients and takes 3-4 minutes to complete.
